Coaching - Individuals, Families and Caregivers
Coaching is a process of working
together with me, to help you gain knowledge and practice effective
communication skills to manage your healthcare or the care of someone
you support and love.
The focus is on
identifying "information gaps" so that when making
an important health related or medical decision, effective communication
can occur. I can coach you regardless of the medical diagnosis,
chronic condition or health related issues you have since I am
coaching you in the area of communication.
The goal is for you
to make the best medical decisions you can. This occurs when you understand
the information you receive and know if more information is needed.
We can be stressed, distracted and overwhelmed when trying to gather
all of the medical information given to us. I can partner with you during
this time so you can participate more fully in your care or the care
of the person you support.

Coaching
Details:
- Sessions range from 45-60
minutes on the phone. Typically I work with clients 2-3 times a month,
but more frequent sessions can be scheduled if requested. Sometimes
one month of coaching brings you to a place where you can communicate
more effectively.
- I will accommodate your
request for an appointment as quickly as possible especially if a
medical crisis has prompted you to contact me.
- I can coach you regardless
of the medical diagnosis or chronic health related issue you have
since I am coaching you in the area of communication.
- I do not give medical
advice. I teach you how to ask for what it is you need
to know, so that you can make the most informed decisions about
your health.

regarding his dad's care and end-of-life options.

learned to coordinate the information from many physicians when her
86 year old father experienced multiple treatments, hospitalizations
and medication changes.

who worried that his early signs of dementia would prevent him from
fully understanding what the doctors were saying.

family history placed him at risk for several medical conditions but
was frustrated his doctor was not "hearing" his concerns

- A brother who was struggling
to absorb and integrate all of the information being given to him
about his sister's stroke, so he could make the best decisions about
her placement in a nursing home.

to be clear, open, organized and sensitive when communicating.

provided via the telephone, but face-to-face sessions are available
upon request. Individual sessions range from 45-60 minutes, 2-3 times
per month.
- Group coaching sessions
with staff or team members if requested are tailored to your specific
needs and are completed on-site. Typically, group sessions are provided
within an educational presentation to set the stage for the coaching
process
